Emergency Medical Information
All students in Tallmadge City Schools must have up-to-date Emergency Medical Authorizations and information through FinalForms. This is completed at the start of every school year or when starting enrollment at TCS.
Students participating in the Tallmadge Blue Devil Marching Band must also have an up-to-date Physical Exam on file in FinalForms.

There are dozens of ways to get involved in the Tallmadge Band Boosters, and we are always looking for volunteers to help provide an amazing experience to our amazing students. Please fill out the form below to let us know how you can use your talents to help the band!
Absence Request Form
In order for an absence to be excused, students must fill out the Absence Excuse Request Form. As soon as you know you will miss an event, please complete this form. In the case of an illness or emergency, please complete it as soon as possible. If possible, a phone call to the band office should be made in the case of illness to 330-633-4561 or an email should be sent to kuhlman.joe@tallmadgeschools.org.
